HealthFirst Family Care Center (Health First) is a Community Health Center that provides primary healthcare to anyone in the Twin Rivers and Lakes Regions of New Hampshire. The five-county area includes 24 rural townships (Franklin, Hill, Bristol, Tilton, New Hampton, Groton, Hebron, Sanbornton, Northfield, Andover, Salisbury, Danbury, Alexandria, Bridgewater, Laconia, Belmont, Center Harbor, Gilford, Gilmanton, Sandwich, Moultonborough, Meredith, and Ashland) in central New Hampshire with a population of approximately 80,000 people.
It is the mission of HealthFirst to provide high quality primary health care, treatment, prevention, and education services required by the residents of the service area, commensurate with available resources, and to coordinate and cooperate with other community and regional health care providers to assure the people of the region the fullest possible range of health services.
We promote health wellness and disease prevention through a variety of health screenings, such as breast and cervical cancer screening. Sore throats, sutures, sprains, strains, digestive problems, depression, diabetes, and physicals are also some of the reasons you may want to see us. We also have an excellent prenatal program.
You, your children, parents, and grandparents now have access to medical care, social services, and nutritional counseling – all in one place for your convenience. Health First can offer a full range of services for your family’s health care needs because we are part of a team with other Health and Human Services providers. Included on the interagency team are Franklin Regional Hospital, VNA of Franklin, Newfound Area Nursing Association, Twin Rivers Mental Health, Genesis Behavioral Health, Community Health and Hospice, LRGHealthcare, Belknap-Merrimack Community Action Program, Child and Family Services, local schools, and other area providers.
HealthFirst is actively working toward certification as a Patient Centered Medical Home.
