Breast and Cervical Cancer
screening- A specialized preventative service offered to women who meet the enrollment
criteria. The breast and cervical cancer screening program provides annual physicals
and appropriate cancer screening procedures.
Primary Care- Primary
and preventative health care services are provided directly by Health First physicians and
mid level providers along with patient case management and nutrition counseling services.
Smoking Cessation-
A program designed to help people stop smoking through a variety of supportive methods.
This program works closely with the prenatal clinic to encourage expecting mothers
to stop smoking
Social Services-
Social services works with many patients on a variety of issues related to health
care and access of services. Workers provide home visits, parenting skills,
enroll people in state funded aid programs, and works with the prenatal team to facilitate
adjustment and positive lifestyle choices for expecting parents. After an initial
assessment of needs/strengths, the social worker can offer a variety of services ranging
from accessing free medications to providing supportive counseling for issues of
depression, grief, loss, adjustment, and abuse.
Nutrition- The nutritionist
provides nutrition counseling in an effort to teach people about the impact of proper
nutrition, and diets specific to health care needs. The primary population seen by
the nutritionist includes patients of the pre natal program, and populations that respond
well to dietary changes such as diabetes patients and heart disease patients.
Prenatal- The prenatal clinic
is designed to offer prenatal services to low income women. Provides a point of
contact and ongoing support for patients in the prenatal program including referrals,
follow up and coordination with other service providers.
Pharmacy Program- Health First
works directly with participating pharmacies to increase the efficiency of prescription
refill, and offers a program of medication at a subsidized rate
Dental Program- Health First
provides dental screenings and education for children in the region, through an outreach
program in local schools for first graders and third graders.
