PRIMARY CARE
Health First Family Care Center provides primary health
care services through an intervention, prevention and education model following that of a
community health center. Services are provided by a full range of health
professionals with the core staff of Health First consisting of an M.D., Nurse
Practitioner, Case Worker, part time Nutritionist, LPN, Part time RN, several part time
nurse's aides, and several part time physicians under contract for special services in the
clinic, including prenatal and pediatrics. All staff are licensed in New Hampshire
in their respective fields.
In office clinical and preventative services:
Within the office, Health First uses a preventative, client
centered model, utilizing an assessment, health screening and preventative service
approach to the family practice of medicine. Clients entering the practice are
assessed for health risks and acute problems, and a care plan is made jointly with the
cine for the delivery of health, social, nutritional and other supportive services.
The clinical staff is organized in service delivery teams to assure continuity of care and
ongoing case coronation. We strongly believe in the team interagency service
delivery network, in a comprehensive programmatic and problem solving model.
