Health First has social workers on staff based at he building at 841 Central Street.  They act as an integral part of the office staff and the community service delivery teams.  The social workers are available to provide case management and outreach services to all clients of Health First Family Care Center and to an extended group of clients connected to Health First through the interagency teams of the Caring Community Network of the Twin Rivers.  This group functions both independently and collectively to perform outreach functions that help any individual in t;he community to gain access to any of the agencies in the integrated service delivery network called the Caring Community Network of the Twin Rivers.
